Guam – In anticipation for the opening of the new school year and increased demand for immunization services, the Department of Public Health and Social Services (DPHSS) will expand these services at the DPHSS Clinic in Mangilao and outreach venues for the period of July 22 to August 12, 2011 according to the schedule below:
• July 22 – DPHSS Clinic in Mangilao, 8:00 a.m. – 11:00 a.m. and 1:00 p.m. – 4:00 p.m.
• July 25 – DPHSS Clinic in Mangilao, 8:00 a.m. – 11:00 a.m. and 1:00 p.m. – 4:00 p.m.
• July 26 – Toto Gardens (DYA BLDG 23)from 9:00 a.m. to 11:00 a.m. or while supplies last
• July 27 – DPHSS Clinic in Mangilao, 8:00 a.m. – 11:00 a.m. and 1:00 p.m. – 4:00 p.m.
• July 28 – DPHSS Clinic in Mangilao, 8:00 a.m. – 11:00 a.m. and 1:00 p.m. – 4:00 p.m.
• July 29 – GHURA Housing in Agat from 9:00 a.m. to 11:00 a.m. or while supplies last
• August 1 – DPHSS Clinic in Mangilao, 8:00 a.m. – 11:00 a.m. and 1:00 p.m. – 4:00 p.m.
• August 2 – Micronesia Mall, 10:00 a.m. to 2:00 p.m. or while supplies last
• August 3 – 12 (except the weekend) – DPHSS Clinic in Mangilao, 8:00 a.m. – 11:00 a.m.
and 1:00 p.m. – 4:00 p.m.
Immunizations will be provided to a limited number of individuals on a first come first serve basis. Parents must bring their child’s most current shot record. All children below 18 years of age must be accompanied by a parent or an adult with written authorization and a valid photo ID.
