The Department of Revenue and Taxation’s Motor Vehicle Division (MVD) announces the release of its Mail-Service Request Form for Guam Driver’s Permits to include Learner’s, Instructional, Motorcycle, Bus, and Tractor-Trailer.
“We want to provide an opportunity for those individuals who have already passed their written exam to be able to request for and receive their permits via the mail,” said Director Dafne Mansapit-Shimizu. “We understand that it has been a challenging time for our people and we are working to find ways to continue to provide services even though we are unable to provide them in-person.”
Upon receiving the completed and signed form together with the total payment (plus postage and handling), required documents, the “Pass” rating, MVD will mail the driver’s permit at the Permittee’s address as indicated in the mail-service form.
Mail-Service Request Form Submission Options
▪ Mail via USPS – Recommendation: Certified with stamped receipt
▪ DRT Drop Box – “Attn: Driver’s License Examination Branch” on enclosed envelope
